Background
In accordance with the Privacy Act, the 多宝游戏下载 maintains information about individuals in its Systems of Records, and permits those individuals to access or correct information about themselves from those systems. A System of Records is a group of any records under the control of the Corporation from which information is retrieved by the name of the individual or some identifying number, symbol or other identifying particular assigned to the individual. Note that not all information is maintained in a System of Records.
Privacy Act Requests
The 多宝游戏下载 processes Privacy Act Requests similar to Freedom of Information Act Requests; accordingly, an individual can submit a Privacy Act Request as outlined in the 多宝游戏下载 Privacy Act Regulations or through the FOIA request process. For each request, please provide the following:
- Proof of Identity鈥 Please provide us with proof of identity to protect the individual to whom the information being sought pertains. 多宝游戏下载 requires a copy of your photo ID, such as a driver's license, AND EITHER
- a Certification of a Duly Commissioned Notary Public, of any state or territory, attesting to your identity
OR - an unsworn declaration subscribed to as true under the penalty of perjury under the laws of the United States of America.
多宝游戏下载 may also require minimal identifying data such as full name, date and place of birth, or other personal information necessary to ensure proper identity before processing requests for records.

Please do not use broad or ambiguous phrases such as 鈥渁ny and all.鈥 If you know when the records were created, tell us; if you鈥檙e not sure, suggest a reasonable time period. - Detailed Background about the Records 鈥 Please include any additional information that will help us to locate the records you seek. You can submit additional documentation.
- Reason for Amendment and/or Correction 鈥 If you are requesting an amendment to a record, please provide a reason for the request.

多宝游戏下载 acknowledges receipt of Privacy Act requests typically within ten business days of receipt, and will notify you whether any System of Records contains a relevant record, and whether the request for access will be granted or denied. If granted records will be disclosed to the individual as soon as practicable. If your request is denied, 多宝游戏下载 will provide you an explanation for denial and outline the procedures you can follow if you choose to appeal the decision.
To appeal a 多宝游戏下载 determination on an access or amendment request, submit your appeal in writing within 30 business days following receipt of notification of denial. Include the following with your appeal:
